Maximize Fall
Prerequisites: Character level 1st, must be afraid of heights
Whenever your character free falls or is hurled to a wall or any obstructing surface you take maximaized damage. This damage cannot be reduced by any means.

Flawed Critical
Prerequisites: Character level 1st, weapon used must not be of masterwork quality.
Whenever you succesfully confirm a critical hit do not roll die anymore instead take the minimun damage your weapon deals and multiply it by our critical modifier.

How about this flaw for casters:

Insomniac
You have trouble sleeping regularly, make a will save to try to fall to sleep and then make a fortitude save to stay asleep. (Since you don't sleep regularly, any day you don't sleep for 8 hours you are counted as fatigued.)

WTF? Are these actual flaws?

Or are they something like so:

CHRONIC GAS EMISSION [FLAW]
You have a problem with your digestive system, in that you pass gas at almost regular intervals. As such, you have become unbearably smelly.
Effect: Whenever you get excited (when successfully attacked, surprised, or as the DM proclaims), you must make a Fortitude save (DC 12 + your Con mod) or you pass gas, filling all squares adjacent to yours with noxious gas. All creatures that start their round in a square filled with gas must make a Fortitude save (same DC) or be nauseated for the duration of the stay in the affected square and for 1d4 rounds thereafter. You get a -4 penalty to all Charisma-related checks.

Prequisite: Character Level 1st
Take a -10 penalty to climb checks. Taking this flaw in conjuncion with maximize fall flaw bestows another free feat.

You are craven. At the beginning of combat, you must succeed on a Will save (DC 10 + CR of the opponent with the highest CR) or immediately flee from battle. If you would attempt to return to battle, you must make a Will save of the same DC as above, albeit at a -10 penalty. You take a -3 penalty to all Cha-related checks when dealing with battle-tested individuals such as fighters, warriors, soldiers, etc. (DM's discretion)

There's a feat in the same name on CHampions of Ruins. +1 per character level on sneak attack damage but -2 on will saves against fear effects.

Clumsy Oaf
Whenever somebody trips you, you automatically fall prone even on a successful check. You also take an additional -2 to ac while prone.
